job description
1. managing clinical outcome of pateints through:
managing issues (preventing peritonitis, infection etc)
measuring outcome of therapy (dor, pr, hr)
regular updating and tracking of pateint status
2.able to communicate at all levels, clinic
a good communicator,
at least 02 year experience as nurse at hospital.etc, on-going care plan and follow up with patient & care givers
draw up the home visits & call visits plans
provide continual education/ training program and support to patient (pd practice knoweldge: exchange procedure, es care, nutrition.provide pre-dialysis counseling
provide efficient, high quality training and support to all pre esrd & esrd patients
enusre suffciient knowledge benig impcated to hd & tranpslant patients
3.)
work cloesly with pd cetner on toatl patient management
review all existing patients who visit their nephrologist & the center at least once a month
4. internal controls and business practices standards and rep office policies
5, to comply with baxter policies.word application is an advantage
able to write/ speak basic english,
practical knowledge of excel.

job requirements
degree in health science, nursing preferred
good clinical knowledge and infection control practices.
